RISE PROGRAMRISE delivers intensive case management services with integrated vocational and educational services, counseling, and support services to eligible, gang-involved, or high-risk youth with the Santa Clara County Juvenile Probation Department.
Eligibility
Youth 12-25, involved with the Santa Clara County Probation Department.
Services
In-home assessment
Case Management
Crisis Intervention
Treatment including medication management and counseling
Education for providers working with youth and their families with behavioral health problems
As needed family therapy
Medication Evaluation and Management
Languages Spoken
