Description and Services:
Intensive case management services are provided to gang-impacted youth and youth involved in the juvenile justice system. Services include weekly individual coaching and counseling, sessions through home visits, school visits, phone check-ins, and advocacy. Case managers help clients establish goals to overcome daily barriers to their success, and provide links to resources for the families, including running parent workshops and advisory sessions. Additionally, after-hours phone support is provided for clients.
- SES partners with the County of Santa Clara Probation Department to provide comprehensive case management services to CRP, EMP, and Probation youth, helping them to meet their court mandates, reduce at-risk behaviors, and build and strengthen their assets.
- Case Managers offer clients the opportunity to contribute to their community through peer teaching.
- The program provides youth with links to educational and vocational information and resources.
- The program provides direct services or links to resources such as literacy/school tutoring, recreation programs, gang prevention and intervention, mental health and substance abuse therapy, truancy intervention, and independent living skills training.
Eligibility and Requirements: Referrals received from the juvenile Probation department.
