Description and Services:
This program is Integrated Services Mentally Ill Parolee contract with the Santa Clara County of the Mental Health Department to serve mentally ill parolee-clients and/or those who are referred from California Department of Correction and Rehabilitation (CDCR).  Program staff will do “whatever it takes” with reasonable on a 24/7 basis to assist the clients. Services include:

  1. Treatment for mental health and substance abuse
  2. Assistance in arranging doctor appointments
  3. Assistance in obtaining the medication needed to maintain good health
  4. Assistance in finding a safe and affordable place to live
  5. Assistance in finding a job and/or returning to school
  6. Assistance in obtaining the financial and or link services that you need
  7. Support available 24/7
  8. Men & Women support group, Dual diagnosis group, Relapse Prevention group, Anger Management group, Living skill group, Awareness group, Education group, and Peer mentor support group, etc.

Eligibility and Requirements:

  1. Adults with mentally ill parolee-clients who are from CDCR.
  2. Partner(s) is referred by Mental Health Court Coordinator on behalf of State Probation.

Waiting List:  None

Fees:  Medical and Medicare accepted

Hours and Availability: Office hours are Monday – Friday 8:30 a.m. to 5 p.m.; but support is available 24/7.
