Job Description:
GENERAL NATURE OF POSITION: As part of the First5 / KidConnections Birth-to-Five Program, the home visitor/case manager provides relationship and evidenced-based early childhood services for young children ages 0-5 and their families to address behavioral health and developmental concerns. The home visitor collaborates with a multidisciplinary team of providers internally and with other providers such as mental health therapists, developmental specialists, teachers, etc. to support the improved functioning of the child, parent-child relationship and the family unit as a whole.

Bi-lingual Spanish is required.


ESSENTIAL JOB FUNCTIONS:

1. Provide home-based rehabilitation and collateral services
• Conduct home visits on a weekly basis for a minimum caseload of 14 families
• Provide face-to-face sessions to assist families in improving the child and caregiver relationship and bond
• Create environments which will assist parents in facilitating their child’s social, emotional, intellectual and physical development
• Provide rehabilitation services at an age-appropriate level to assist the client in learning skills to improve mental health functioning
• Assist parents in learning new skills and addressing barriers to positive parenting and building healthy parent/child bond
• Utilize assessment tools including KIPS, CANS: EC, CBCL 1.5-5, C-TRF 1.5-5, ASQ-3. ASQ-SE, PRQ and PSI
• Have understanding of diagnosis using DC:0-3R and DSM-5
• Develop treatment plans with child and family that utilize core values of Transformational Care Planning (TCP)
• Record services, maintain client files, write progress notes and complete monthly activity reports for UNICARE, and AWARDS
• Adhere to Medi-cal documentation and protocol standards and timelines
2. Provide case management services (meets with families in the community or home settings)
• Provide parent education and linkages to needed community services to promote healthy social, emotional and physical development for each child
• Collaborate with other professionals within the KCN network or other agencies to provide the most individualized and appropriate care for the client and their family
3. Provide Triple P Positive Parenting services:
• Facilitate a minimum of two Triple groups each fiscal year
• facilitate Triple P standard sessions on a case-by-case basis, as needed
4. Participate in reflective individual and group supervision
5. Participate in required KCN meetings and trainings
6. Attend required Catholic Charities trainings, meetings and events
7. Complete all other duties as assigned

Education and Experience:
• At minimum, a BA in counseling, psychology, child development, occupational therapy or related field, or a minimum of five years of experience in the early childhood mental health field
• Experience in First 5 system and/or mental health services in Santa Clara County a plus.

Required Knowledge, Skills & Abilities:
• Bilingual Spanish/English
• Case management/counseling experience in a community-based setting
• Effective verbal and written communication skills
• Understanding and sensitivity of economic, cultural issues in diverse communities
• Ability to work independently with minimum supervision while in the field
• Ability to multi-task and meet deadlines
• Demonstrate computer literacy (Client electronic records system, MS Word, Excel and Internet Usage)
• Team-oriented with strong interpersonal and organizational skills
• Fingerprinting and background check as required by the DOJ and DMV clearance
• TB test required

• Fast-paced, diverse and dynamic environment
• Must be able to work within emotionally charged therapeutic sessions, self-reflect on own triggers and reactions, seek appropriate supervisory support, and engage in adequate self-care activities
• Must be able to report to service site on a regular and reliable basis
• Must be able to work a flexible schedule, which includes day, evening, weekend and holiday hours as appropriate, to coordinate and provide services and activities for family during non-traditional hours
• Able to travel between sites and to attend off-site events throughout the county, as appropriate

Licenses, Certifications, Registrations & Other Qualifications:
• Must possess a valid California driver’s license, reliable transportation and current auto insurance
• Must be at least 18 years of age
• Must be a U.S. citizen or lawful permanent resident

Hiring Range: Contingent on license, education, experience and language skills

ABOUT KCN SYSTEM OF CARE: The KidConnections Network (KCN) of Providers is a collaboration between Santa Clara County Mental Health Department and FIRST 5. Through the utilization of home visitors, therapists and developmental specialists, we provide comprehensive developmental screening and assessment, behavioral health services, Triple P Positive Parenting, and connection to specialize educational programs and resources. Our specialized team of Home Visitors and Early Childhood Mental Health Therapists are here to support the health development of children through age 5, and enrich the lives of their families and communities.
