GENERAL NATURE OF POSITION: The Restorative Justice Program helps our communitys most venerable residents heal their mental health and access various social supports to stabilize their lives for recovery and successful reintegration into the community. The Therapist provides holistic services, including individual and group rehabilitation services, therapy, and case management to assist mental health, substance abuse and dual diagnosis restorative justice clients. S/he assists clients to access medical, educational, social, prevocational, vocational, rehabilitative or other needed community services for eligible individuals. Operates as a Licensed Clinician in the program. Provides crisis intervention services for clients in various situations and does involuntary hospitalizations. Works with interdisciplinary treatment team to assist the clients in their recovery and management of chronic mental illness, substance use, chronic health disorders and compliance with court requirements. Provides clinical supervision of unlicensed case managers and assists Senior Division Director with other aspects of program management on an as needed and requested basis.
ESSENTIAL JOB FUNCTIONS:
Is the lead therapist for client assessments and treatment plans for the program.
Provides behavioral health services to an intensive caseload of 14 to16 seriously mentally and/or dually diagnosed ex-offender clients with co-occurring disorders and documentation process.
Participates in a team environment to provide 24/7 client support, may be part of the after-hours on call rotation list.
Ensures that the clients feel welcomed and engage in the program.
Ensures that clients are connected to primary care and substance abuse counseling.
Maintains and/or exceeds consistent productivity targets.
Coordinates services with psychiatrist, team members, primary care and specialist physicians, legal representatives/guardians, Departments of Mental Health and Alcohol and Drug staff, probation, parole, District Attorney, pre-trial services, Public Defender and other community services/organizations to assure the individuals access to services, the services, and the service delivery system.
May assist clients in completing applications for services, assistance, ID cards, etc.
Connects clients with NAMI, AA, NA and other supportive organizations to assist the clients in their recovery.
Follows Mental Health Department documentation standards and documents all activities according to program requirements.
Records services in Electronic Health Record (EHR) system, maintains client files and completes monthly activity reports.
May provide outreach activities at the Santa Clara County jails and occasional outreach activities to screen potential clients who have been referred through the Mental Health Department.
May review progress notes of assigned staff as indicated.
May provide supervision for the program and/or assist another program.
Performs related work and other duties as required.
Required and Preferred Education and Experience:
Licensed degree in social work, psychology, or marriage and family therapy.
Experience working with low-income individuals with serious mental illness and ex-offender background, including providing services in community-based settings.
Good oral and written English skills.
Culturally competent communication skills.
Ability to work well within a team environment.
Santa Clara County Department of Mental Health documentation experience preferred.
Computer skills, Microsoft Office and web-based application.
Ability to lift 25 pounds
Valid California drivers license and personal automobile.
Liability insurance according to agency requirements.
Must be able to pass DOJ clearances upon employment, facilitated by CCSCC.
Must be able to pass DMV clearance.
Have an NPI number.
