The Manager provides leadership and oversight of Catholic Charities of Santa Clara County Day Break Home Care Program. The Manager is a member of the Older Adult Services leadership team and is responsible for the program and business development of Day Break Cares Home Care Services.
The Manager will provide leadership in the development and oversight of communications strategies and marketing campaigns for Day Break Cares. As a key member of the Day Break Cares staff, the manager is responsible for expanding the organizational reach to community stakeholders and earned income opportunities.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Program and Staff Development: Responsible for the supervision, evaluation, and day to day management of the home care team. Provide leadership and guidance for the use of personnel and non-personnel resources. Effectively collaborate with other systems and agencies to develop and maintain a network of services for current and emerging needs of seniors. Develop methods of standardizing services, programs, and protocols across Day Break Cares. Foster the development and maintenance of highly functioning, ethnically and culturally diverse teams.
Business Development: Support the overall process of management and division decision-making to ensure the organization maximizes its short, medium and long-term profitability. Liaise with other team members on the implementation of CCSCC strategic and operational plans. Achieves targeted outreach goals necessary to support program objectives and promote business growth. Communicates effectively with program personnel to coordinate growth activities. Actively participates in Day Break Cares business growth meetings and strategic planning meetings. Effectively cultivates new business opportunities. Participates in effective networking activities. Promotes customer service with timely call backs, effective listening and understanding.
Client Care Coordination: Oversight of Home Care Program quality assurance and care coordination services. Oversight of scheduling systems and staff to ensure client satisfaction and accuracy in billing. Responsible for scheduling procedures including: determining staff availability, matching appropriate direct care worker to meet the needs of individual clients, communicating basic care plan and contact information to assigned direct care worker, following up with client and worker regarding care services.
Communications & Marketing: Ensure efficient and effective marketing, advertising and promotional planning with the help of OAS and CCSCC marketing team.
Customer Service: Ensures client satisfactions with services; Attracts potential clients by answering service questions. Resolves service problems by clarifying the clients complaint; determining the cause of the problem; selecting and explaining the best solution to solve the problem; expediting correction or adjustment; following up to ensure resolution.
Other duties as assigned.
Education: (Equivalent education and experience that demonstrates the ability to perform the job may be considered.)
1. A minimum of 10 years experience working with in a home care setting.
2. B.A in health, social services field. LVN or RN Required. MBA, MPH preferred.
Experience with data input on computers
Demonstrated commitment to and competence of managing confidential information and files
Demonstrated ability to work in racially diverse communities
Required knowledge, skills & abilities:
Good oral and written communication skills; ability to adapt delivery for audiences of varying ages, cultures, and economic and educational backgrounds
Must be detail-oriented and organized to manage complex scheduling demands of caregivers and clients
Flexibility and willingness to take on new tasks
Demonstrated ability to work in an environment that promotes compassion, service, and social justice
Commitment to and support for program development and organizational initiatives
CPR and First Aid certified required
Ability to stand and be active for 2-3 hours at a time
Use a keyboard, computer terminal, telephone, and other office equipment
Occasionally lift or move up to 20 lbs.
Hours and other conditions:
Available to be on-call after hours and on weekends
