GENERAL NATURE OF POSITION: The program coordinator renders daily operational assistance, coordinates Step Up SV activities, communicates and coordinates with collaborators, and completes various administrative duties. This will include working with the members of 1000 Out of Poverty, a cluster of SUSV, including case managers, Community Technology Alliance, and other community supporters. The coordinator will maintain team activities and prepare monthly reports through BaseCamp, Googledocs, and in-person meetings.

The Registered Nurse, as a professional practitioner in the primary and behavioral health care integrated clinic (clinic), assumes responsibility and accountability for the application of the nursing process and the delivery of patient care including performance of office procedures and clinical procedures under the direction of the Supervising Physician and specific performance procedures of the clinic. Makes clinical judgments in an effective and efficient manner with supervision; demonstrates critical thinking and ability in the coordination of patient care. Provides primary health care services to participants.
