CORAL After-School Instructor

 

General Function:

 

Under the direct supervision of a Site Manager, the CORAL After-School Instructor (CASI) will be responsible for the supervision and safety of approximately 20 school age children.  Within a small group setting the After School Instructor will help develop and carry out lesson plans, and work to raise academic achievement within a balanced literacy model.

Community Health Nurse LVN / Case Manager

GENERAL NATURE OF POSITION: Provides case management/brokerage for older adults to access medical, educational, social, prevocational, vocational, rehabilitative or other needed community services for eligible individuals. Provides basic nursing services, social services, and wellness education of the individual clients participating in the program. Communicates with primary care and intern specialists as required to facilitate disease management. Works with peer family mentors to assist the clients in their recovery and management of chronic mental illness, substance use and other chronic disorders.. In the spirit of the program, “do whatever it takes” to assists the client in accomplishing his/her goals.

1. Transport our elderly clients to/from their homes to/from the program site as needed on Monday, Wednesday and/or Thursday as needed in a prompt, safe, and friendly manner, using the wheelchair accessible van provided by the agency.

2. Demonstrate excellent customer service skills.

3. Demonstrate professional driving skills at all times.

4. Assist the elderly Golden Gateway clients with getting in/out of the van as needed.

5. Fill the agency van with gasoline as needed.

6. Conduct regular inspections and report any vehicles faults to the agency’s Facilities Manager.

Licensed Clinical Social Worker Therapist
